   Clinic General Information

   • Please make corrections, sign and return Students Emergency/Safety Card.
   • All Students are required to have a complete Gerogia Certificate of Immunization
      (Form 3231).
   • School Medication Policy will be followed for all medications (prescribed and over
      the counter) given during school hours.

    Over the counter medication authourization must be signed by parent/legal guardian.
      The clinic strongly encourages parents/legal guardians to send in Tylenol or Advil for
       students to have for headaches, minor aches or cramps during the school year.

   • Prescribed medications authorization mus t also be signed by a physician as well as
      the parent /legal guradian. (School Medication Authorization Form).
   • Health Care Plans are needed on all students with severe allergies, asthma,
      diabetes, seizures or other chronic health problems. Please have healthcare
      provider complete and sign care plan.
   • The main reasons for keeping your child home are he/she's too sick to participate
      comfortable at school or he/she might spread a contagious disease to other
      students.
   • Students should stay home from school if there is :

  • Fever above 100.4
  • Vomiting
  • Diarrhea
  • Frequent cough
  • Persistant pain (ear, stomach, etc.)
  • Widespread rash
  • Head lice or nits
  • Bacterial Pink eye (until under treatment)
  • Feminine products are available for 0.25 each.

Clinic is open 8:00am - 4:00pm and is located next to commons area in room 3095. Please contact your school clinic if you have any questions or concerns at 770-460-3540 ext 121.
